THE COMPLAINT FORM

This form may be used to submit a complaint to me. There are several important points which you should note about its use:

If you do not wish to use the complaint form, you may write a letter of complaint instead. If you do so please ensure that you include all of the information listed at "What do I do next?"

• The form may be completed on-screen. Once completed the form should be printed off and the declaration signed by hand. Please note that, for security reasons, we cannot accept this form by e-mail.

• Alternatively the form may be printed off and filled in by hand.

• If you do not have access to a printer but you wish to complete a complaint form please contact the office and ask for a form be sent to you.

• The form must be signed.

• The completed form should be sent to me through a MLA if your complaint is against one of the bodies within my jurisdiction as Assembly Ombudsman

• The completed form should be sent directly to me if your complaint is against one of the bodies within my jurisdiction as Commissioner for Complaints

• If you have any copy papers relevant to your complaint please enclose them with your complaint form.
