THE COMPLAINT FORM

This form may be used to submit a complaint to me. There are several important points which you should note about its use:

Complaint Form in Word (.doc) Format

Either print off and filled in by hand

or complete on-screen, then print off and sign by hand

If you do not wish to use the complaint form, you may write a letter of complaint instead. If you do so please ensure that you include all of the information listed at "What do I do next?"

Completion of the form

The form may be completed on-screen. Once completed the form should be printed off and the declaration signed by hand. Please note that, for security reasons, we cannot accept this form by e-mail.

• Alternatively the form may be printed off and filled in by hand.

• If you do not have access to a printer but you wish to complete a complaint form please contact the office and ask for a form be sent to you.

• The form must be signed.

Where to send the completed form

The completed form should be sent to me through a MLA if your complaint is against one of the bodies within my jurisdiction as Assembly Ombudsman

The completed form should be sent directly to me if your complaint is against one of the bodies within my jurisdiction as Commissioner for Complaints

What to send with the completed form

If you have any copy papers relevant to your complaint please enclose them with your complaint form.
